Informed Pregnancy: MCH-Registry goes mobile

This project is an extension of the existing MCH-Registry of the Swiss Tropical and Health Institute, which is an open source web-based software tool that supports health care providers in managing pregnant women. The system is in use in 17 Ukrainian hospitals. Pregnancy related information is collected for visits to the facility, tests, medication prescribed, hospitalizations, and as well as on delivery and newborn.

The extension sends notifications to patients, doctors and hospital staff based on this data. The seven message types sent by this application include visit reminders, recommendations, discharge notifications and monthly reports. Having 10,000 to 15,000 pregnancies in 17 Ukrainian hospitals yearly, 1.5 million notifications a year are expected.

The project consists of two parts: The SMS Service, which is designed to reach patients, doctors and hospital staff on simple mobile phones and the Pregnancy Guide mobile application for Android using Google Cloud Messaging which is directed to patients only. It is the choice of the patient which one to use.

For both parts, notifications are fed in a queue by a cron job and sent to patients and hospital staff either by SMS (delivered by an SMS provider) or a server-side Java application using Google Cloud Messaging messages (upstream and downstream). Patients using the mobile application benefit from extra information like the expected delivery date, details about the registering facility, can keep a library of the received recommendations and can take notes of happenings during the pregnancy which they would like their doctor to know. Administrators and facility users are able to configure the outgoing notifications per facility by a web interface (PHP, MySQL, jQuery). Keywords in the freely editable message templates can be used for the name of patients, doctors, facilities, scheduled days etc. The amount of recommendations and their sending day during a pregnancy is free for configuration. Message types can be enabled and disabled and configured for the whole application, at facility and at pregnancy level. The system supports retrospective sending an creation of notifications in case of a failure.

The web application supports English, Ukrainian, Russian and Romanian while the Android app supports English and Ukrainian.

The notification service may enable better quality of antenatal- and perinatal care and a reduction of neonatal and maternal mortality by reminding doctors and patients of upcoming appointments and giving women advice during a pregnancy. Prior, doctors had to periodically check for new problematic cases in the database.
